Introduction & ideal-model criteria

  • Why screen — peptic ulcer disease results from an imbalance between aggressive factors (acid, pepsin, H. pylori) and defensive factors (mucus, bicarbonate, prostaglandins, mucosal blood flow); a candidate anti-ulcer drug may act on either side, so a battery of complementary models — not a single assay — is used.
  • Mechanisms a screen must detect — the clinically validated targets — H2-blockers, proton-pump (H+/K+-ATPase) inhibitors, prostaglandin analogues, antacids and mucosal protectives — define the acid-dependent, cytoprotective, neurogenic and ischaemic pathways an ideal panel must cover.
  • Criteria of an ideal experimental ulcer model — simple, reproducible and easily quantified; works across species; produces characteristic ulcers at a defined site (stomach vs duodenum) through a distinct mechanism; ulcers do not heal spontaneously within the observation window; cost-effective and not unduly time-consuming.
  • Two-tier philosophy — in-vitro mechanistic/target assays (receptor binding, enzyme inhibition) form the first high-throughput filter; in-vivo whole-animal ulcer models then confirm efficacy, define the protective mechanism and generate dose–response data.
  • Parietal-cell basis — the parietal cell is driven by three secretagogues — histamine (H2), acetylcholine (M3) and gastrin (CCK2); histamine is the final common mediator, and H+/K+-ATPase (the apical proton pump exchanging H+ for K+) is the terminal, mechanism-convergent step that makes the pump the most downstream screening target.
