Introduction

  • Antimalarials are drugs used for the prophylaxis, treatment, and prevention of relapse of malaria, acting on one or more stages of the Plasmodium life cycle in the human host.
  • Malaria is caused by protozoa of the genus Plasmodium; five species infect humans — P. falciparum, P. vivax, P. ovale, P. malariae, P. knowlesi. P. falciparum causes most severe disease and deaths; P. vivax dominates in South Asia and is increasingly a cause of severe malaria.
  • Global burden ~a quarter-billion cases and >600,000 deaths/year; malaria remains a top-five killer of children <5 yr. Two clinical problems dominate the modern exam: drug-resistant (chloroquine-resistant, multidrug-resistant) P. falciparum and cerebral malaria.
  • India / NCVBDC context: CQ-resistant P. falciparum is now widespread (no fully sensitive area), so national policy is ACT for all P. falciparum plus single-dose gametocidal primaquine; the falling case trend is attributed to effective ACT use.
